ACTIVITY AIDE
The primary purpose of this position is to assist with the planning, implementation and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan and as may be directed by your Supervisor.

COOK I
The primary purpose of this position is an entry-level cook’s position for the beginning cook who is willing to take instruction in cooking techniques and kitchen skills.  This position will work under the general supervision of the Cook II & III, the Sous Chef, and the Executive Chef.  The person in this position will prepare and serve food products for the residents and customers. The position will follow proper procedures for receiving food and food storage; will perform assigned sanitation duties, assigned record keeping duties, and other duties as assigned.

DIETARY MANAGER/SNF
Under the guidance of the DDS, the Dietary Manager is responsible for planning, organizing, developing and supervising the overall operation of the Dietary Department in the skilled nursing facility in accordance with current federal, state, and local standards, guidelines and regulations to assure that quality nutritional services are provided on a daily basis and that the dietary department is maintained in a clean, safe, and sanitary manner.  The Dietary Manager is responsible for operating the department utilizing the principles and mission and values of O’Connor Woods, the development and training of staff, and enhancing the dining experience of the residents.

Dietary Services Supervisor Certification or equivalent (Bachelor’s Degree in Nutrition with a minimum of 1 yr experience in a Skilled Nursing setting) required; Dietary Services Supervisor experience preferred
HS Diploma required and some college preferred or equivalent combination of education and/or experience
Ability to read, speak and write English and to follow oral and written directions
Must possess a thorough knowledge of dining service operations and supervision
Must be ServSafe certified
Minimum of two (2) years management/supervisory experience in health care or residential facilities required
Knowledge in MDS is required
Must be familiar with Title 22 and CMS guidelines
Must possess a comprehensive knowledge of serving techniques, resident relations, sanitary and safe food handling and preparation procedures, plate presentation, personnel administration and supervision
Strong interpersonal and written/oral communication skills required to tactfully relate with resident seniors and staff, including the occasional challenging situations
Must be flexible with scheduling according to the needs of the department
Ability to prepare a variety of tasteful meals and to follow prepared menus and portion control guides
Ability to understand the therapeutic diets and their application to the written menu and to oversee the preparation of it.


HOUSEKEEPER
According to established procedures, provides a thorough cleaning of all cottages, apartments and common areas to promote resident independence.  Demonstrates positive interpersonal relationship with supervisor, co-workers and other personnel.  Maintains communication with residents, staff and co-workers.  Minimum six (6) months experience in similar residential or commercial housekeeping environment

LVN
The licensed vocational nurse has the responsibility of ensuring timely interventions and treatments, providing for patient comfort, safety and personal care.
This position requires the full understanding and active participation in fulfilling the Mission of the Organization. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support the Organization’s strategic plan and the goals.  Ability to read, speak and write English and to follow oral and written directions
Current license as Licensed Vocational Nurse by California Board of Examiners of Vocational Nurses.
Ability to supervise certified nursing staff.
Ability to participate in continuing education to maintain license.
Ability to deal with residents, families and physicians courteously and professionally.
CPR certification.
Ability to participate as a team member in the planning, delivery and evaluation of patient care.
Minimum of one (1) year experience in a Skilled Nursing Facility is preferred.



RN CHARGE NURSE
The registered nurse has the responsibility of ensuring timely interventions and treatments, providing for patient comfort, safety and personal care for assigned residents.  The registered nurse is also responsible to serve as a resource for other licensed nursing staff on duty in all clinical areas and is expected to function as the lead nurse in the absence of the RN Supervisor, Assistant Director of Nursing and the Director of Nursing.

The employee shall support the Organization’s strategic plan and the goals.  Current California RN License and CPR certification required.

DRIVER - PART TIME
Possession of Class B or higher Driver's License with Passenger Endorsement
Minimum of one to two (1-2) years commercial or bus driving experience. 
Excellent driving record. 
First Aid certified.
Ability to speak, read and write English and to follow simple written and oral instructions.
Ability to read/understand driving directions, routes and traffic signs.
Patience and skills in dealing with the elderly under sometimes trying conditions.
Will be required to have a Department of Transportation physical, and random drug testing


RN- Skilled Nursing
